WHAT IT DOES
UnitedHealth runs two businesses that feed each other. UnitedHealthcare is the largest private health insurer in America. Optum is everything around the insurance: OptumHealth employs or affiliates with tens of thousands of physicians, OptumRx negotiates drug prices as one of the three big pharmacy benefit managers, and OptumInsight runs the billing and claims plumbing much of the industry depends on — including Change Healthcare, the clearinghouse whose 2024 ransomware attack froze claims payments nationwide.
HOW IT MAKES MONEY
Premiums come in through the insurer; a striking share of the money then flows to businesses UnitedHealth also owns — its own PBM fills the prescriptions, its own clinics and physician groups deliver the care, its own data arm processes the claims. Every internal handoff keeps margin inside the house that would otherwise be paid to someone else. That loop is the entire design.
WHY WE TRACK IT
You cannot understand American healthcare — a fifth of the U.S. economy — without understanding this company, because it sits on both sides of most of the transactions. It is also the cleanest test of whether vertical integration in healthcare survives its political moment: the same loop that makes the margins makes the target.
THE SIMPLE THESIS
UnitedHealth built the most complete toll system in American healthcare, collecting at every gate a patient's dollar passes through. The thesis question is no longer whether the machine works — it demonstrably does — but whether regulators, legislators, and Medicare's actuaries let it keep working on the same terms.
